Background. Forms V2 is the new paperwork system for client visits. During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care / Annual (SOC/AO) visit on site. This issue is one item from their feedback (item D4 in
docs/ActionItems.md
).

The problem

Some questions on the paper ask several things in one sentence, e.g. the 3040's "Is a change needed in tasks or hours, or should services end?", which is three questions. One yes/no answer can't say which part it means.

What to do

The 3040 case is handled by H5 (#95): ask only about tasks or hours, in a set format.

Go through every interview step and list the other compound questions.

For each, propose: split into separate questions (each printing to the right box), or narrow it as H5 does. Send the list to Jane to confirm.

Note: this has to sit alongside G1 (#77, use the paper's wording). Where we split a question, each part still carries the paper's question number.

"}
