During a visit, staff go through an on-screen interview with the client, the system fills in the agency&#39;s paper forms, and each signer signs on screen. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item <strong>W4<\/strong> in <code>docs\/ActionItems.md<\/code>).<\/p>\n<h2 id=\"user-content-the-customer-s-rule\" dir=\"auto\">The customer&#39;s rule<\/h2>\n<p dir=\"auto\">Witnesses are needed <strong>only when the client signs with an &#34;X&#34;<\/strong> (cannot write their name). Then <strong>two<\/strong> witnesses sign. Otherwise there are no witnesses.<\/p>\n<h2 id=\"user-content-how-it-works-today\" dir=\"auto\">How it works today<\/h2>\n<p dir=\"auto\">Witness questions and signature lines are offered whatever way the client signs.<\/p>\n<h2 id=\"user-content-what-to-do\" dir=\"auto\">What to do<\/h2>\n<ul dir=\"auto\">\n<li>Add (or reuse) a question: &#34;Will the client sign with an X?&#34;<\/li>\n<li>Yes: require two witnesses (name + signature, date stamped automatically per W3).<\/li>\n<li>No: hide the witness questions and leave the witness lines blank on every form.<\/li>\n<\/ul>\n<h2 id=\"user-content-technical-notes\" dir=\"auto\">Technical notes<\/h2>\n<p dir=\"auto\">Elements: <code>form.*.witness_1<\/code>, <code>form.*.witness_2<\/code> and their signing slots.
