From Jane (7 Oct 2026), about the Patient Information & Emergency/Disaster Preparedness Plan:

This form bothers me. The question about special needs is a check box, yes or no. What's the point of asking that question if you don't know what the special needs are. Think I'm going to ask about this because they go crazy preparing for extreme weather conditions. Seems as though knowing what the special needs are might be helpful.

The paper defines special needs as "one who needs assistance during evacuation and sheltering because of physical or mental handicaps or one who requires a level of care and resources beyond the basic first aid level of care that is available in shelters for the general population."

Jane is asking the customer. If they agree, options:

Add a follow-up "What are the special needs?" shown when the answer is Yes (a list, plus free text), kept with the visit even if the paper has no line for it; or

Fill it from what we already know (the Class I–IV category further down the same form, equipment, ADLs).

Related: #141 (prefill this form's check boxes), #100 (STEAR registration).

"}
