During a visit, staff go through an on-screen interview with the client, and the system fills in the agency&#39;s paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item <strong>S3<\/strong> in <code>docs\/ActionItems.md<\/code>).<\/p>\n<h2 id=\"user-content-what-the-customer-said\" dir=\"auto\">What the customer said<\/h2>\n<p dir=\"auto\">&#34;How often will the attendant be supervised&#34; is always <strong>every 12 months<\/strong>. On paper it reads &#34;Every 12 months \/ PRN&#34; (PRN = as needed). Remove the question.<\/p>\n<h2 id=\"user-content-how-it-works-today\" dir=\"auto\">How it works today<\/h2>\n<p dir=\"auto\">It is asked on the service plan, and the same idea is asked again for:<\/p>\n<ul dir=\"auto\">\n<li>the PAS service plan (<code>form.pas-eval-service-plan.frequency_of_supervision<\/code>, <code>_months<\/code>, <code>_other<\/code>)<\/li>\n<li>the PCS consent (<code>form.consent-pcs.supervision_interval<\/code>)<\/li>\n<li>the private pay agreement (<code>form.private-pay-agreement.supervision_interval<\/code>)<\/li>\n<li>the 3040 (<code>form.forms-3040.frequency_of_visits<\/code>)<\/li>\n<\/ul>\n<h2 id=\"user-content-what-to-do\" dir=\"auto\">What to do<\/h2>\n<p dir=\"auto\">Remove the question and print &#34;Every 12 months \/ PRN&#34; (or each form&#39;s equivalent box) on each of those forms.<\/p>\n<h2 id=\"user-content-waiting-on\" dir=\"auto\">Waiting on<\/h2>\n<p dir=\"auto\"><strong>Confirm with the customer (via Jane)<\/strong> that the same default applies to the consent forms, the private pay agreement and the 3040, not only the service plan.
