Background.<\/strong> Forms V2 is the new paperwork system for client visits. During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue comes from Frank's notes of that meeting (item K2<\/strong> in docs\/ActionItems.md<\/code>).<\/p>\n

Where we are<\/h2>\n

The Other care the client receives<\/em> step (Coordination of Care: SN \/ HHA \/ other providers and their schedules) and the service plan's "other services" (form.pas-eval-service-plan.service_other_services<\/code>, \u2026other_services_na<\/code>, \u2026other_services_receiving<\/code>) are asked separately. The same information is collected twice, and the two can disagree.<\/p>\n

What to do<\/h2>\n

  1. Decide which one is asked and which is worked out. Probably: Coordination of Care is asked, and the service plan's other-services line is derived from it ("None" when no provider is entered).<\/li>\n
  2. Rule it in code\/form_v2_shared.inc.php<\/code> (shared, or derived) so test-form-v2-shared.php<\/code> records the decision.<\/li>\n
  3. This answers S5 (#90): what prints when there are no other services.<\/li>\n
  4. While here: form.pas-eval-service-plan.other_services_receiving<\/code> is currently labelled "What change is needed, and why?" and shown only when the 3040 change_needed<\/code> = yes (SOC_AO.json, Health, home and needs<\/em> step).
