During a visit, staff go through an on-screen interview with the client, and the system fills in the agency&#39;s paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item <strong>W5<\/strong> in <code>docs\/ActionItems.md<\/code>).<\/p>\n<h2 id=\"user-content-what-the-customer-said\" dir=\"auto\">What the customer said<\/h2>\n<p dir=\"auto\">When &#34;I am not providing a record&#34; is ticked, <strong>the box below it should be ticked automatically<\/strong> as well. The two always go together.<\/p>\n<h2 id=\"user-content-what-to-do\" dir=\"auto\">What to do<\/h2>\n<ol dir=\"auto\">\n<li>Find the pair on the paper. Most likely the advance directive section: <code>shared.advance_directive.providing_copy<\/code>, on form H2076 or the advance directive acknowledgement.
