I'm leaving this open until Jane confirms the list below.\n\n**How it works:** there is a new interview setting, `starts_as`. When a visit is started, the question already holds that answer, and the record's own value wins if it has one. The screen says under the question: \"Starts as Yes. Change it if that is not what happened.\" That way it stays clear the supervisor is attesting to it. A pre-set answer that is changed never counts as a difference from VESTA.\n\n**Starts as Yes:** these are the things the supervisor did.\n- Disaster plan: went over the agency policies\n- Task Guide: went over the program's scope, the safety instructions, and when to call the supervisor\n- Things You May Not Do: explained that services are only for the client\n- 3040 orientation: the client's health condition; tasks, schedule and safety; reporting events to the supervisor; the complaint procedure given and explained\n- PAS\/PHC and PCS consents: went over the rights\n- PAS\/PHC consent: client told in writing and in person what the attendant can and cannot do\n- Private Pay: attendant instructions; the Obligation of Client and terms\n- Advance directives: client received the agency's information\n\n**Left blank on purpose:** these record what the client decides or says, and a default there would be the agency answering for the client.\n- Consent to services (all three consents)\n- \"Has the client reviewed and approved the service plan\" (PAS\/PHC and Private Pay)\n- 2-1-1: \"Does the client understand them?\"\n- 3040: \"Has the attendant had HIV \/ hepatitis training?\" (a fact about the attendant, not something explained at the visit)\n\n**Jane:** please confirm both lists.
