During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue comes from Frank's notes of that meeting (item **K1** in `docs\/ActionItems.md`).\n\n## What the customer said\n\"Provide Directory with Agency Names and Phone Numbers\/Fax Numbers.\"\n\n## Where we are\nOther providers are typed in by hand: *Other care the client receives* (Coordination of Care: skilled nursing, home health, other), and possibly the Transfer form.\n\n## What to do\n1. Ask where it is used: Coordination of Care, Transfer, or both. Ask whether the customer has a list.\n2. Store the directory. It is not PHI. A new table or a file, and it needs a way to keep it up to date.\n3. In the interview, pick from the directory (with an \"Other\" choice). Picking fills the name, phone and fax.\n4.
