It fills the Coordination of Care and both pages of the service plan (`shared.other_services` in `code\/form_v2_shared.inc.php`). The choices are None, skilled nursing, home health aide, therapy, dialysis, adult day care (DAHS), medical equipment (DME), meals (HDM), emergency response, transportation and other.\n\n**What each paper ticks.** These need the customer's eye:\n- **Coordination of Care:** SN, HHA, therapy, dialysis, adult day care (as ADC) and other. It has no rows for equipment, meals, emergency response or transportation, so those don't print there. Each row is another agency's care with one contact, and its single Other row can't hold several services. \"None\" prints nothing ticked.\n- **Service plan, page 1** (\"Other Service Individual is Receiving\"): nursing and aide both tick **Skilled Home Health Aide**. Therapy, dialysis and equipment tick **Other**, and its \"Other (specify)\" line is asked. None ticks **None**.\n- **Service plan, page 2** (\"Other Services\"): meals tick **HDM** and None ticks **N\/A**. Anything without its own box ticks **Other**, and \"Other (specify name)\" is asked.\n- The adult day care's agency name is also page 2's DAHS name, so it's asked once.\n\nAll the follow-up questions now sit under the list on one step, and the overlap question is hidden when the answer is None. This also settles S5 (#90): no other services prints **None** and **N\/A**.\n\n**Item 4, the suspected mis-binding: not a fault.** Every interview version (1\u20138), the form definition and the placement bind `other_services_receiving` to page 1's \"Other Service Individual is Receiving\". \"What change is needed, and why?\" is the 3040's `change_needed_detail`. It's the question shown when `change_needed` = yes, on the *Attendants* step.\n\n**To ship:** the drafts are imported on dev and the publish rehearsal passes all nine checks.
