It fills the Coordination of Care and both pages of the service plan (<code>shared.other_services<\/code> in <code>code\/form_v2_shared.inc.php<\/code>). The choices are None, skilled nursing, home health aide, therapy, dialysis, adult day care (DAHS), medical equipment (DME), meals (HDM), emergency response, transportation and other.<\/p>\n<p dir=\"auto\"><strong>What each paper ticks.<\/strong> These need the customer&#39;s eye:<\/p>\n<ul dir=\"auto\">\n<li><strong>Coordination of Care:<\/strong> SN, HHA, therapy, dialysis, adult day care (as ADC) and other. It has no rows for equipment, meals, emergency response or transportation, so those don&#39;t print there. Each row is another agency&#39;s care with one contact, and its single Other row can&#39;t hold several services. &#34;None&#34; prints nothing ticked.<\/li>\n<li><strong>Service plan, page 1<\/strong> (&#34;Other Service Individual is Receiving&#34;): nursing and aide both tick <strong>Skilled Home Health Aide<\/strong>. Therapy, dialysis and equipment tick <strong>Other<\/strong>, and its &#34;Other (specify)&#34; line is asked. None ticks <strong>None<\/strong>.<\/li>\n<li><strong>Service plan, page 2<\/strong> (&#34;Other Services&#34;): meals tick <strong>HDM<\/strong> and None ticks <strong>N\/A<\/strong>. Anything without its own box ticks <strong>Other<\/strong>, and &#34;Other (specify name)&#34; is asked.<\/li>\n<li>The adult day care&#39;s agency name is also page 2&#39;s DAHS name, so it&#39;s asked once.<\/li>\n<\/ul>\n<p dir=\"auto\">All the follow-up questions now sit under the list on one step, and the overlap question is hidden when the answer is None. This also settles S5 (#90): no other services prints <strong>None<\/strong> and <strong>N\/A<\/strong>.<\/p>\n<p dir=\"auto\"><strong>Item 4, the suspected mis-binding: not a fault.<\/strong> Every interview version (1\u20138), the form definition and the placement bind <code>other_services_receiving<\/code> to page 1&#39;s &#34;Other Service Individual is Receiving&#34;. &#34;What change is needed, and why?&#34; is the 3040&#39;s <code>change_needed_detail<\/code>. It&#39;s the question shown when <code>change_needed<\/code> = yes, on the <em>Attendants<\/em> step.<\/p>\n<p dir=\"auto\"><strong>To ship:<\/strong> the drafts are imported on dev and the publish rehearsal passes all nine checks.
