During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Some answers open further questions: answer \"Yes\" to *Does the client have a POA?* and the POA details appear. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **D2** in `docs\/ActionItems.md`).\n\n## The customer's concern\nIf someone goes back and changes a primary answer after its follow-up questions are filled in, the follow-ups disappear and the forms change, possibly without anyone noticing.\n\n## How it works today\nThe server decides which questions show. Changing a primary answer hides its follow-ups (and their answers stop printing). Nothing warns or locks.\n\n## Options\n1. **Lock**: once follow-ups are answered, the primary answer can't be changed (or only by a supervisor).\n2. **Confirm and clear**: changing it asks \"This will remove N answers below. Continue?\" and clears them if confirmed.\n3. **Warn only**: show a notice that follow-up answers were hidden.\n\nOur suggestion is **2**.
