During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **A6** in `docs\/ActionItems.md`).\n\n## Jane's question\n3040 **Q8** reads \"Attendant is\/continues to be competent to provide\u2026\". Where in the visit is that answered?\n\n## The answer\nIt is asked in the **Attendants and supervision** step as \"Is Attendant A\/B\/C competent to do the tasks?\", one question per attendant (`form.forms-3040.competent_a`, `_b`, `_c`).\n\nIt was hard to find because our label doesn't match the paper's wording.
