During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **G4** in `docs\/ActionItems.md`).\n\n## Why\nThe customer wants each question labelled with the form's own wording (G1). Our design notes still say the interview uses a spoken, plain-English rewording. Anyone (or any AI session) that writes interview questions from those notes would keep doing the wrong thing.\n\n## What to do\n- Update FormsV2Plan \u00a74.1.2: a question's label is the paper's wording. The rule that a form's explanatory text (consent paragraphs, rights, etc.) must also show on screen still stands.\n- Update the matching project memory (`feedback_v2_placement_by_claude`) and the README in `docs\/form-v2-interviews\/`.\n\nThis is a documentation change.
