During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **A1** in `docs\/ActionItems.md`).\n\n## What the customer said\nOn the 3040, **question 3b and question 7 always have opposite answers**. If 3b is Yes, 7 is No, and the other way round.\n\n## What to do\n1. Find which interview elements print at 3040 Q3b and Q7. We haven't mapped the paper's question numbers yet (see G2, #78).\n2. Ask only 3b. Q7 is worked out as its opposite (a \"derived\" value) and is no longer a question.\n3.
