During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue comes from Frank's notes of that meeting (item **H9** in `docs\/ActionItems.md`).\n\n## What the customer said\n\"Schedule Note: Pulls from Service Plan Start Date.\"\n\n## Where we are\n`form.forms-3040.schedule_fwd` is labelled \"Schedule note (FWD)\" and asked as free text (\"Copy what the schedule says here\u2026\"). If it is a date, our reading of the field is wrong.\n\n## What to do\n1. Check the 3040 paper: what \"FWD\" is and what format it expects.\n2. Prefill it from the service plan start date (`auth.service_start_date`, or whichever element the PAS evaluation \/ service plan prints) and stop asking it.
