When a visit is started, the question already holds that answer, and the record&#39;s own value wins if it has one. The screen says under the question: &#34;Starts as Yes. Change it if that is not what happened.&#34; That way it stays clear the supervisor is attesting to it. A pre-set answer that is changed never counts as a difference from VESTA.<\/p>\n<p dir=\"auto\"><strong>Starts as Yes:<\/strong> these are the things the supervisor did.<\/p>\n<ul dir=\"auto\">\n<li>Disaster plan: went over the agency policies<\/li>\n<li>Task Guide: went over the program&#39;s scope, the safety instructions, and when to call the supervisor<\/li>\n<li>Things You May Not Do: explained that services are only for the client<\/li>\n<li>3040 orientation: the client&#39;s health condition; tasks, schedule and safety; reporting events to the supervisor; the complaint procedure given and explained<\/li>\n<li>PAS\/PHC and PCS consents: went over the rights<\/li>\n<li>PAS\/PHC consent: client told in writing and in person what the attendant can and cannot do<\/li>\n<li>Private Pay: attendant instructions; the Obligation of Client and terms<\/li>\n<li>Advance directives: client received the agency&#39;s information<\/li>\n<\/ul>\n<p dir=\"auto\"><strong>Left blank on purpose:<\/strong> these record what the client decides or says, and a default there would be the agency answering for the client.<\/p>\n<ul dir=\"auto\">\n<li>Consent to services (all three consents)<\/li>\n<li>&#34;Has the client reviewed and approved the service plan&#34; (PAS\/PHC and Private Pay)<\/li>\n<li>2-1-1: &#34;Does the client understand them?&#34;<\/li>\n<li>3040: &#34;Has the attendant had HIV \/ hepatitis training?&#34; (a fact about the attendant, not something explained at the visit)<\/li>\n<\/ul>\n<p dir=\"auto\"><strong>Jane:<\/strong> please confirm both lists.
