During a visit, staff go through an on-screen interview with the client, and the system fills in the agency&#39;s paper forms from the answers. Form 3040 is the state&#39;s supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item <strong>A6<\/strong> in <code>docs\/ActionItems.md<\/code>).<\/p>\n<h2 id=\"user-content-jane-s-question\" dir=\"auto\">Jane&#39;s question<\/h2>\n<p dir=\"auto\">3040 <strong>Q8<\/strong> reads &#34;Attendant is\/continues to be competent to provide\u2026&#34;. Where in the visit is that answered?<\/p>\n<h2 id=\"user-content-the-answer\" dir=\"auto\">The answer<\/h2>\n<p dir=\"auto\">It is asked in the <strong>Attendants and supervision<\/strong> step as &#34;Is Attendant A\/B\/C competent to do the tasks?&#34;, one question per attendant (<code>form.forms-3040.competent_a<\/code>, <code>_b<\/code>, <code>_c<\/code>).<\/p>\n<p dir=\"auto\">It was hard to find because our label doesn&#39;t match the paper&#39;s wording. That is the general problem fixed by G1 (#77).<\/p>\n<h2 id=\"user-content-to-do-before-closing\" dir=\"auto\">To do before closing<\/h2>\n<ul dir=\"auto\">\n<li>Check that <code>competent_a\/b\/c<\/code> is the field actually printed at 3040 Q8 (proof the PDF).<\/li>\n<li>Send Jane the answer.<\/li>\n<\/ul>\n"}
