During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **A2** in `docs\/ActionItems.md`).\n\n## What the customer asked\nIn the 3040's orientation section (\"attendant should report problems to the agency\"), **remove** the questions \"who should they report to\" and \"phone number\". They are always the agency itself.\n\n## What to do\nFill both from the agency's own name and phone number (the agency, or the branch if each branch has its own phone). Stop asking them.\n\n## Check\nIs the phone the agency's main number or the branch's?
