The national registry lists agencies with their phone **and fax** numbers, and a weekly update keeps it current, so there is no list for the office to maintain.\n\n**Where the lookup appears:**\n- **Coordination of Care:** each agency under \"Other care the client receives\" has a **Find the agency** button that fills the agency's name, phone and fax:\n - skilled nursing and home health aide: home health and home care agencies;\n - therapy: therapy clinics and home health agencies;\n - dialysis: dialysis centers;\n - adult day care: adult day care centers.\n- **Transfer form:** the agency the client is transferring from and the agency called. The form has no phone or fax boxes, so only the name is filled.\n\nAgencies this agency has picked before come first, marked \"Used before\". The contact person is still typed, because the registry doesn't list people at an agency.\n\n**For the customer:**\n1. Is putting it on both forms right?\n2. If they have their own list, it can be loaded into the same table alongside the registry.\n3. \"Home health\" here also includes in-home support agencies, where most PAS providers are registered.
