During a visit, staff go through an on-screen interview with the client, the system fills in the agency's paper forms, and the client signs on screen. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **D1** in `docs\/ActionItems.md`).\n\n## The customer's question\n> \"Mistake on a form after the fact: how do they change it?\"\n\n## Where we are\n- **Before signing:** any answer can be changed. Saving a change replaces the review copy the client is shown.\n- **After signing:** the visit takes no more answers. The signed PDF is kept as the record and can't be altered. **There is no way to correct it yet.** (The old Forms V1 had an \"amend\" flow, retired on 1 Oct 2026.)\n\n## What needs designing\nA correction that **never changes the signed original** but produces a new, linked revision. To decide:\n- who may correct (a right, e.g. the `Amend` right that already exists in our permission list),\n- whether the correction has to be signed again by the client,\n- how the corrected version and the original are shown and kept (both kept, with who changed what and why),\n- whether a correction is per form or re-opens the visit.\n\n## Answer for Jane now\nMistakes can be fixed any time before signing. After signing there is no correction path yet.
