None of the others numbers its questions, so for those forms no number is shown, and none is made up.\n\n**On the 3040** the numbers are items 1, 2A, 2B, 2C, 3A, 3B, 4\u20139 and 10-1 to 10-7. They are recorded for all 96 of its numbered fields. The visit screen shows them beside the question, e.g. **\"Form 3040, item 8\"**. The form itself calls them \"items\" (\"items 4 and\/or 5\"). A question that fills several numbered forms would show each one.\n\n**How it's stored:** a `numbering` block in `docs\/form-placements\/forms-3040.json`, carried into the form's map, so the numbers are pinned with the paper. A new revision of the form can renumber it. The Form Explorer shows the number on each box row.
