During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms (Form 3040, the PAS service plan, the consent forms and so on) from the answers. The client then signs on screen. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **G2** in `docs\/ActionItems.md`).\n\n## What the customer asked\n> \"Also need question numbers that are on form.\"\n\n## How it works today\nQuestions on screen carry no reference to the paper. Staff can't tell that a question is, say, question 3b on the 3040.\n\n## What to do\nShow the form and its question number beside each question, e.g. \"3040 \u2014 Q3b\".\n- The placement files (which say where each answer prints on the PDF) need a `number` for each field. None has one today, so each form's numbering has to be read off the paper and recorded.\n- A question that fills several forms shows each form's number.\n\n## Technical notes\nPlacements: `docs\/form-placements\/`. Once this exists, add the numbers to the data-element list (R1) and to the Form Explorer (`\/admin\/form-explorer`).
