During a visit, staff go through an on-screen interview with the client, and the system fills in the agency&#39;s paper forms and produces PDFs for the client to sign. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item <strong>T4<\/strong> in <code>docs\/ActionItems.md<\/code>).<\/p>\n<h2 id=\"user-content-the-bug\" dir=\"auto\">The bug<\/h2>\n<p dir=\"auto\">On the generated <strong>task guide<\/strong> PDF, the agency <strong>logo<\/strong> and the <strong>bottom line (footer)<\/strong> each appear <strong>twice<\/strong>.<\/p>\n<h2 id=\"user-content-what-to-do\" dir=\"auto\">What to do<\/h2>\n<p dir=\"auto\">Not looked into yet. Likely causes to check:<\/p>\n<ul dir=\"auto\">\n<li>the blank PDF template already carries the logo\/footer, and our placement draws them again, or<\/li>\n<li>the page is stamped twice (e.g. once in the review copy, once at signing).<\/li>\n<\/ul>\n<p dir=\"auto\">Generate a task guide in dev with <code>form-pdf-proof.php<\/code>, compare it with the blank template, and fix the placement or the render.
