Background.<\/strong> Forms V2 is the new paperwork system for client visits. During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item H5<\/strong> in docs\/ActionItems.md<\/code>).<\/p>\n

What the customer asked<\/h2>\n

The 3040 asks "Is a change needed in tasks or hours, or should services end?"<\/em>. That is three questions in one.<\/p>\n

  • Ask only about tasks or hours<\/strong> (drop "should services end" from the visit).<\/li>\n
  • The answer must be in a set format<\/strong>, not free text.<\/li>\n
  • Answering "yes, a change is needed" does not edit the hours<\/strong>. It files a change request to the coordinator (#94, H4).<\/li>\n<\/ul>\n
