During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue comes from Frank's notes of that meeting (item **K2** in `docs\/ActionItems.md`).\n\n## What the customer said\n\"Other care the client receives <-> Service plan.\"\n\n## Where we are\nThe *Other care the client receives* step (Coordination of Care: SN \/ HHA \/ other providers and their schedules) and the service plan's \"other services\" (`form.pas-eval-service-plan.service_other_services`, `\u2026other_services_na`, `\u2026other_services_receiving`) are asked separately. The same information is collected twice, and the two can disagree.\n\n## What to do\n1. Decide which one is asked and which is worked out. Probably: Coordination of Care is asked, and the service plan's other-services line is derived from it (\"None\" when no provider is entered).\n2. Rule it in `code\/form_v2_shared.inc.php` (shared, or derived) so `test-form-v2-shared.php` records the decision.\n3. This answers S5 (#90): what prints when there are no other services.\n4. While here: `form.pas-eval-service-plan.other_services_receiving` is currently labelled \"What change is needed, and why?\" and shown only when the 3040 `change_needed` = yes (SOC_AO.json, *Health, home and needs* step). That looks like a mis-bound element.
