During a visit, staff go through an on-screen interview with the client, the system fills in the agency's paper forms, and each signer taps to sign on screen. Each signature line's date is filled automatically with the date of the tap. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **W3** in `docs\/ActionItems.md`).\n\n## What the customer asked\nDate the **witness** signatures automatically, like the other signature lines.\n\n## How it works today\nThe witness date is asked as a question instead of coming from the witness's signing tap.\n\n## What to do\nTreat the witness date lines as part of the witness signing slots, so the date is stamped when the witness signs, and remove the witness date questions.\n\n## Technical notes\nElements: `form.*.witness_1_date`, `form.*.witness_2_date`. A date line claimed by a signing slot is never a question (`form_v2_signed_fields()`), so this is mostly a placement change.
