During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **H6** in `docs\/ActionItems.md`).\n\n## What the customer said\nOn the 3040, \"Does the client continue to need the services?\" is **always Yes**.\n\n## Decision needed from the customer\n- **Default to Yes but still show it**, so staff can change it in the rare case it's No, or\n- **remove the question** and always print Yes.\n\n## Technical notes\nElement: `form.forms-3040.continues_to_need_services`.
