During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Form 3040 is the state's supervisory visit form. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **H5** in `docs\/ActionItems.md`).\n\n## What the customer asked\nThe 3040 asks *\"Is a change needed in tasks or hours, or should services end?\"*. That is three questions in one.\n- Ask **only about tasks or hours** (drop \"should services end\" from the visit).\n- The answer must be in a **set format**, not free text.\n- Answering \"yes, a change is needed\" **does not edit the hours**. It files a change request to the coordinator (#94, H4).\n\n## How it works today\nA yes\/no question plus a free-text detail box.\n\n## Decision needed from the customer\n**What is the set format?** For example: Change needed: None \/ Tasks \/ Hours \/ Both, then for tasks which ones added or removed, for hours the requested new weekly hours.
