During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. Many of the state forms carry the agency's **vendor number** at the bottom. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **F1** in `docs\/ActionItems.md`).\n\n## What the customer said\nThe vendor number on the bottom of the forms must be checked. Sometimes it should be the **contract number** instead, and the number **depends on the branch**.\n\n## Decision needed from the customer\n1. **The rule**: on which forms (or for which programs\/payers) is it the vendor number, and when the contract number?\n2. **The numbers**: the vendor and contract numbers for each branch of each agency.\n\n## Then\nStore the numbers per branch (not typed in each visit) and print the right one by the rule.
