During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms from the answers. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **S3** in `docs\/ActionItems.md`).\n\n## What the customer said\n\"How often will the attendant be supervised\" is always **every 12 months**. On paper it reads \"Every 12 months \/ PRN\" (PRN = as needed). Remove the question.\n\n## How it works today\nIt is asked on the service plan, and the same idea is asked again for:\n- the PAS service plan (`form.pas-eval-service-plan.frequency_of_supervision`, `_months`, `_other`)\n- the PCS consent (`form.consent-pcs.supervision_interval`)\n- the private pay agreement (`form.private-pay-agreement.supervision_interval`)\n- the 3040 (`form.forms-3040.frequency_of_visits`)\n\n## What to do\nRemove the question and print \"Every 12 months \/ PRN\" (or each form's equivalent box) on each of those forms.\n\n## Waiting on\n**Confirm with the customer (via Jane)** that the same default applies to the consent forms, the private pay agreement and the 3040, not only the service plan.
