During a visit, staff go through an on-screen interview with the client, and the system fills in the agency's paper forms and produces PDFs for the client to sign. On 29 Sep 2026 Jane showed the customer the Start of Care \/ Annual (SOC\/AO) visit on site. This issue is one item from their feedback (item **T4** in `docs\/ActionItems.md`).\n\n## The bug\nOn the generated **task guide** PDF, the agency **logo** and the **bottom line (footer)** each appear **twice**.\n\n## What to do\nNot looked into yet. Likely causes to check:\n- the blank PDF template already carries the logo\/footer, and our placement draws them again, or\n- the page is stamped twice (e.g. once in the review copy, once at signing).\n\nGenerate a task guide in dev with `form-pdf-proof.php`, compare it with the blank template, and fix the placement or the render.
